Critical fumbles

What do you guys tend to do for critical failures on attack rolls? I find 1 and 2 dice to be pretty easy, slowed for a round or maybe sprawled on 1 dice something like that, 2 dice stuns or disarms the target (the disarmed weapon is not easily retrieved). 3 and 4 are where I stumble. What kinds of things do you use for those or have used in the past?

3: Hit a buddy, or Fling the weapons 1d6 squares in a random direction.
4: Hit a buddy with no DR, or Weapon auto fails 1 damage save.

My basic scale is that each die spent should be equivalent to 1 lost action or 1d6 points of damge, usually subdual representing muscle pulls or sprains.

1- loss of one half action
2- loss of one full action and flat footed
3- bleeding, fatigued, shaken or blinded, entangled, slowed (the last ones for a round or two)
4- as above plus something else, honestly it's hard that players invest those many dice in a fumble :D

I quite often find myself without inspiration and resort to a table i've created where I can roll % dice and give myself some assistance. It's cross referenced by the number of action die/dice so I can have a variety of outcomes to choose from. I'd be happy to post if anyone wants to look at it
