A typical fantasy campaign features a lot of ‘dungeon crawls’. A Huge or Gargantuan PCs is effectively barred from dungeon adventures. My campaign will not have dungeons. There will be overland exploration, politics, clashes on battlefields, romance, enormous monsters, and struggles between faiths. If you are running a campaign such as this that suits bigger PCs you could consider opening up Truly Massive allowing PCs to about 30’ tall. However for my campaign set in a mythic first age the gigantes, titanic 60' giants who once defied the gods, still stride across the world.

Interesting and similar. However, I do still see the place for "dungeon crawls" in even this style of game. Note however, I've never been a huge fan of dungeon crawls to begin with. They should be used on a  limited basis in any campaign, sort of a "break" from the more normal adventures you find every day around the PCs. 

In a world like mine where "powerful" and "large" creatures rule, there are still ruins of one form or another. These races have large lifespans and the dominant races even have records of watching what I call the Hominid races evolve from primates into what they are now (pechs, humans, lycans and orcs). Over all of those Millenia there have been many wars, many destroyed cities, destroyed temples, etc. Beyond that even are the more primitive ruins of where the first Jotun and Giants emerged into the world from the Aether. So yes there are the options for 'dungeon crawls'. I just don't make them common (or easy). 

As for the size disparity... simply think of the scenes from LotR in the Mines of Moria. (In fact thats actually pretty spot on for my world, where dwarves (who are elemental jotunbrood kin) have been dominated (even enslaved) by their Jotun cousins. Those scenes would be the "go-to" scene for any mining operation in the Jotun Kingdom.

On the topic of Dungeon Crawls, to me at least in an 'epic' game the 5 room dungeon principle becomes more appropriate than ever.
I can't think of anything in books or film where characters have to trawl through a D&D style dungeon. Massive sprawling architecture has it's place, but even in Moria it was a travel scene not a crawling scene.

Back when the book was new I ran a game that had a Truly Massive Drake in it (because, you know, OMFGBBQ PLAYER DRAGONS!!!) and he suffered through a lot of the "How do I function in this scene?".  It's hard to put a cost on a balance factor like that, especially one that would be utterly moot in many a game (like many of the game ideas in this thread).

In terms of breakdown, they get (large to Huge):
+50% Con to Wounds
+2 Grapple (second+ check), Bullrush, Trip,
+5 Str worth of carrying capacity
+Natural Attack (boosted damage from size)
-1 Defence
-4 Blend and Sneak

Pretty huge (get it?) benefits really, but structually identical to the boost from medium to large.  If it wasn't for the fact that I don't see players gravitating towards large races, my gut reaction would be that Large is worth more then 1pt.  Of course double something is usually not worth just double cost (Thick Hide 2 = 1pt, Thick Hide 4 = 3pts), but honestly it's the wounds that are the big one.  With a low point of ~20 wounds, you will be all but immune to instadeath by critical.  I'd be inclined to call it a 3pt ability (a butch feat, but doable with a moderate downside), but obviously I'm not seeing something that Pat does - which makes me wonder what that is?  I'm not doubting, just curious what I'm missing there (entirely because I'm pondering huge for some ingame applications).


Also, side thought - Legendary Drake / Rootwalker / Giant?  Sure that puts you at a minimum of like 10-14th level to attain it, but it would certainly be a suitable part of that kind of class.  Plus I can't be the only one who misses those Master Classes, can I?
