So the electrical thing in his chest is keeping the bits of shrapnel from sliding in and shredding his heart.  When he comes back to the states, why not have them taken out.

Looking at the shrapnel we're shown in the cave, I'd say that the point behind the weapon is that the fletchets are deliberately too small to be removed without potentially causing more damage.

Well, there's also Jarvis, but the very fact that Jarvis is an AI rather than some queeny old buttler is meant to demonstrate that apart from Pepper and Rhodry, Tony really has no meaningful human contact with anyone else.
